Key Responsibilities


Formulate, implement, and continuously revise clinical plans of care in collaborative partnership with attending physicians and multidisciplinary healthcare teams.


Provide direct, skilled home-based nursing interventions and comprehensive patient assessments (OASIS), including systematic medication reconciliations.


Oversee and supervise technical care provision executed by home health aides and licensed practical/vocational nurses in alignment with state and federal guidelines.


Coordinate, document, and sync patient visit details and care plans in complete compliance with payer mandates and strict agency timelines.


Actively engage in health service quality initiatives, survey readiness procedures, utilization management, and targeted measures to reduce unnecessary patient re-hospitalizations.


Provide timely communications regarding clinical adjustments, care evaluations, scheduling dynamics, and coordination workflows to agency personnel, patients, and families.



Experience & Qualifications


Active, current, and completely unrestricted Registered Nurse (RN) licensure in the state of Pennsylvania.


Minimum of 1+ years of professional nursing experience specifically within a Home Health clinical setting.



Current, verified CPR Certification.


Valid Driver's License, active automotive insurance, and reliable access to personal or public transportation.


Complete physical and professional capacity to function effectively in varied, independent home environments across diverse patient demographics.



Technical Skills Required


Deep proficiency in specialized Home Health nursing interventions and infection control protocols.


Mastery of OASIS data collection, comprehensive clinical evaluation processes, and medication reconciliation standards.


Strong capabilities in utilizing mobile clinical documentation software and healthcare scheduling platforms.


Knowledge of healthcare utilization management standards and local/state/federal home health regulations.
